NEON (Northwick & Ealing Outreach Network)
Holistic support for young people admitted to the clinical wards at Northwick Park and Ealing Hospitals.
NEON is an innovative care model inspired by the successful Kings Adolescent Outreach Service (KOAS). Its primary goal is to provide an additional layer of holistic support to young people aged 16-24 admitted to the clinical wards at Northwick Park and Ealing Hospitals.
This service is facilitated by dedicated youth workers from Brent, Ealing, and Harrow, who conduct thorough assessments, promote health, and provide guidance on local support services. Additionally, NEON offers community follow-ups when necessary.
NEON youth workers are committed to visiting and supporting every young person admitted beyond A&E, based on referrals from hospital staff. This proactive approach ensures that each patient receives comprehensive care throughout their hospital stay, addressing their individual needs and fostering overall wellness.
The role of the youth worker encompasses outreach efforts, providing essential support during hospital stays, advocating for patient care, and connecting young individuals to local services that can assist them post-discharge.

Project Criteria:
- Young people aged 16-24 admitted to Northwick Park and Ealing Hospitals
- Admission to a non-Paediatric ward
- No specific restrictions based on the reason for admission
